ISC for ME, LLC

The ISC for ME, LLC team supports people to identify their medical, employment, financial, social, educational, and other goals, identify the services necessary to meet those needs including natural supports in the community and waiver providers, learn how to advocate for their goals and facilitate access to identified services to live as independently as possible in their community. 


  The team at ISC for ME, LLC believes in and promotes the dignity, worth and diversity of all people.  

The role of a community case manager (CCM) as a team member is support people live as independently as possible while assuring their health and safety requirements are met. This is a 4 step process. CCMs support people to:

1.     Identify their needs and desires

2.     Advocate for what they want

3.     Find resources they need

4.     Secure resources to meet their needs and desires 

The team at ISC for ME, LLC strives is to engage each person we provide services to in the step by step process known as the Person Centered Process (PCP) to assist each person to identify their goals based on their needs and desires.
